Re-Enrollment For Returning Students
Intent To Re-Enroll For 2010-2011 School Year - A decision will need to be made and the re-enrollment form turned into the school by February 18, 2010. If the form is not returned by February 18, 2010, it is determined that your student's space at ALA is unclaimed and will be offered to a new student wishing to enroll.
We are accepting applications for the 2010-2011 school year. Please see the front desks for a tour and information about the programs being offered at ALA. We are a Title One school and therefore can offer additional services for our students. Contact ALA for additional information.

ALA Admissions Policy - this form explains the Admissions process for getting your student into ALA or re-enrolling them, along with deadline dates for current and new students.
Application for Waitlist - Completion of this application will give you a spot on ALA 's waitlist. Once an opening in our enrollment occurs, we will refer to our waitlist and notify by phone and/or mail the students who are first in line on the waitlist. Being placed on the waitlist does not guarantee enrollment at American Leadership Academy .

Immunization Requirements - this form covers the various required immunizations as well as the Utah state codes regarding immunizations and exemptions.
